Qualifications

  • A BA/BS in Behavioral Health, Social Work, or a related field.

  • At least 3 years' experience in care coordination or clinical practices.

  • Preferred - Certified Alcohol and Drug Counselor-II

  • Must have a compassionate nature, be knowledgeable about substance use disorders and mental health care, and provide exceptional customer service.

  • Excellent communication, organization, time management skills and the ability to work effectively both individually and as a member of a team.

  • Competent in risk assessment, safety plans, crisis intervention, behavior management, case management and patient care coordination.

Job Summary

The Behavioral Health-Care Coordinator is a core member of the Residential Treatment Team and is responsible for supporting the coordination, engagement and follow-up in clinically indicated behavioral and physical health services for a caseload of up to 24 adolescent clients in the residential treatment program. They regularly consult with clients and their families to determine their needs, coordinate services, educate them about their conditions and options, and evaluate progress and interventions. They also participate in regular care coordination team meetings with educational, medical, psychiatric, other behavioral health and juvenile justice providers and communicate resulting treatment recommendations to the Residential treatment team.
